Our Constituent Card provides you with the opportunity for first hand trips to Chicago's Museums, Zoo, Gardens, and Aquariums. With over 15 million visitors per year, these remarkable institutions offer visitors priceless opportunities to explore and enjoy the arts, sciences, nature, and culture.

• Please complete the form one time per visit request -- if you wish to request two dates, please fill out the form twice.
• The form must be completed in full in order to receive the tickets.
• You will receive the ticket information via the email address supplied here.
• Please ensure you have a three possible preferred dates for your visit.
• Ticket requests must be made two weeks prior to your anticipated visit date. Our office cannot process same day ticket requests.

Important notes:

1. Card only grants holder of the card for ONE DAY USE.
2. We require at least 2 weeks notice for the request to be fulfilled.
3. General Admission Tickets Only- no event admission or parking is included.
4. You must state how many ADULTS and CHILDREN are attending. No more than four people can attend for free.
5. The person filling out this form must be in attendance and show ID at the entrance.

While we share your excitement about the newly opened Obama Presidential Center, we want to clarify that our office cannot supply free tickets to this museum. While the museum requires ticketed admission, they do have free day options available on their website.
